The text is blank

Date Time was blank. The action returns blank Formatted Date Time and does not fault. Map the Date/Time field, or add a Decision before the action when a missing time should take another path.

The Flow faulted

Time Zone Offset was blank, outside -14:00 to +14:00, had invalid minutes or seconds, or was not a recognized time zone ID. A name such as Pacific is not an ID. Use America/Los_Angeles, or a fixed offset such as -07:00.

Put a fault path on the action when the zone comes from a field someone can leave empty or mistype.

Daylight saving did not apply

A fixed offset never changes. -07:00 stays seven hours behind UTC in January and in July. Use a named time zone when the local clock should follow that zone’s daylight-saving rules.

The AM/PM or month name looks unexpected

Those pieces follow the running user’s language. The pattern letters are still the Java date/time pattern you set. h is the 1–12 hour, and H is the 0–23 hour.
